In Search of: An "about me" kit of quick pages for child

I recently saw a digital scrapbook kit or an example that someone did for their child to help them learn phone numbers, address, etc. It was a really great idea. I can't remember where I saw it.

I'm looking to either do one myself or buy the kit. Can anyone help me find the website? Even other examples of pages for kids like that might help too. THANKS!
