Here's how I make my buttons...I'm sure there's a shorter way to do it but this works for me.
1. Create a new file: File > New > 500x500 pixels, 72dpi, raster background, transparent > OK
2. Create a new vector layer: Layers > New Vector Layer > OK
3. On the vector layer, you will be creating a circle using the Preset Shape Tool. Choose the circle. Create as vector and AntiAlias should be checked; retain style should be unchecked. Your foreground color should be transparent (no color). Click the background color. Switch the tab to Gradient. Choose the Sunburst style. Click on the dropdown and choose any of the options that include "Duotone" in the style (ex: Duotone dark blue). Click OK.
4. Create the circle: Hold shift key down, drag a circle to fill most of the area.
5. Center the button: Objects > Align > Center in Canvas.
6. Deselect the circle: Selections > Select none.
7. Create a second vector layer.
8. Change the background color from the sunburst gradient to a solid color very different from your circle's color (black or white is fine).
9. Create one of the button holes: Use the Preset Shape Tool to create a circle the size you want your button hole to be (shift then drag). The location doesn't matter at this point...we'll move it later. Do not deselect.
10. Copy the buttonhole: Make sure the vector2 layer is still highlighted and your buttonhole is still selected. Edit > Copy.
11. Create a third vector layer.
12. Edit > Paste > Paste As New Vector Selection. Place it anywhere on your button.
13. Deselect the second buttonhole.
14. Position the buttonholes: Click on the Object Selection Tool then select one of the buttonholes.
15. With the buttonhole selected and using the Object Selection Tool, drag the top buttonhole where you want it to appear. Do not deselect.
16. Align the top buttonhole: Objects > Align > Horiz. Center in Canvas.
17. Repeat with the second buttonhole but this time position it below the top buttonhole.
18. Align the bottom buttonhole: Objects > Align > Horiz. Center in Canvas.
19. Deselect.
20. Convert all the Vector layers to Raster layers: Click once on Vector 1 layer > Right click > Convert to Raster Layer. Repeat for all three vector layers.
21. Click once on Raster 3 Layer which should be one of your buttonhole layers.
22. Using the Magic Wand, highlight the buttonhole only (do not include any of the background).
23. Delete the buttonhole. The buttonhole should be gone but the selection should still be there...DO NOT DESELECT YET!
24. Click once on Raster 2 Layer (which should be your button layer).
25. Delete the selection on your button layer. Deselect.
26. Select the other buttonhole and repeat steps 22-24.
27. Layers > Merge > Merge Visible.
28. You should be able to resize this as needed using the Deform Tool (right click and drag to keep the button's perspective.) Be sure to add a drop shadow. (Try offsets of "0", opacity 72 and blur 10 and color black.) Also, the button can be colorized to suit your layout (Adjust>Hue and Saturation>Colorize).
