i haven't sent in my shaker box tutorial yet, i'll try to do that asap. not sure about the shape problem. can you make a square one? if so, simply use another shape for the top layer. my tutorial will be for any shape as the steps would be no different really. can you be more specific? thanks!

use custom shapes to make the frame (duplicate layer to create a background- just change the color of the first layer).

for the cut out- use the custom shapes tool. make your shape, then ctrl + click (on pc) on the shape layer to select it, then go back to the frame layer layer, and hit delete on your keyboard. delete the layer that you used to create the selection, so that it is no longer there.

Talking Shaker box Shapes

I have seen shaker boxes in heart shapes, animal, flower. But I read on a site that shapes other than circle or square were more difficult to make. But sometimes that can be depending on the person too, lol.
