In your File Prefernences> File locations do you have your Sub-folder box checked? Try that and see if the folder within the folder show up.

Are you trying to save the same shape again, such as the heart and that is why you are seeing it in the Create Brush Tip window? Try a different image and make a selection and see if it creates a brush from your new selection. (You don't have to save this, click cancel)
This would be handy if you had say a image/photo and you want to select a certain area of that photo to turn into a brush. It's really not nessecary to learn this method but I wanted to point out that it is there and handy in some situations.
I am heading to work in a bit today but I think it will be slow enough and I will be able to get online.
