Open a new document 3 inches by 3 inches - 300dpi. Create a solid color shape on a new layer.

Double click on the layer with your shape to open the layer style window. Set the Inner Shadow, Bevel and Emboss/Contour, and Satin styles as follows:

Your final product should look like this:
